Requirements To Reinstate Service (Fraud Terminations Only)




IP address used to create fraudulent accounts will be permanently banned. In order to reinstate service, you must fax the following proofs to 1.888.219.7378:

- Valid Phone Number
- Valid First/ Last Name
- Valid State
- Valid City
- Valid Zip
- A valid photo ID with the above credentials
- The front of the card used to purchase the service

Again these items must be faxed to 1.888.219.7378, afterward you must call our technical support department to verify your contact information, at which point we will re-instate service

We apologize for the great extent for verification, but these would be the sufficient proofs required to re-instate service.
